I probably haven't played enough games, but System Shock 2 is still the "scariest" game for me.

Aliens vs. Predator 1 and 2 come pretty close.

I've never played Resident evil or system shock [i really want to play it but it seems to have disapprered from existance]
for some reason I found Doom 3 and Aliens vs Predator 2 very disappointing in terms of scariness.

the one game that has scared the crap out of me the most is
The White Chamber, a game by indie developers Studio Trophis. it's a point and click sci-fi horror game with an animé style to it. it's probably not up to par with the likes of resident evil, but it spooked me good.

Metroid Prime is definately a scary game for me. I always HATED going to the pirate's bases! That ominous drone hum scared the crap outta me.
